Report as to Affairs and Presentation of Summary of Affairs of a company:

All directors of companies that enter into administration or liquidation are required to complete a Report as to Affairs detailing the company's assets and liabilities. Accordingly, if you are a director of a company in administration or liquidation, please complete this form and return same to us attaching all relevant supporting documentation. The Presentation of Summary of Affairs of a company is required when preparing to place a company into Creditors Voluntary Liquidation.

Proof of Debt Form:

If you are owed money by a company in administration/liquidation, please download and complete this form and return to us to register your claim as a creditor. Please ensure all relevant supporting documentation is included with the form.

Appointment of Proxy Form:

If you are unable to attend a meeting of creditors that is being held for a company of which you are a creditor, and you would like someone to attend on your behalf, please download and complete this form and return to us by no later than 4pm on the day prior to the meeting. If someone is unable to attend the meeting on your behalf, but you would like your interest noted in the meeting, you are able to appoint the Chairman of the meeting as your proxy.
